Hillcrest Center has the following  job opportunities at the W. J. Rolark Short Term Family Housing site!

Case Manager

Hours- Tuesday-Saturday 11:00am-7:00pm

The Case Manager works directly with the Lead Case Manager and the Social Work team to help families residing at the Short Term Family Housing facility connect with services. The case manager will utilize a multi-disciplinary approach to assist residents by providing outreach and intensive case management, perform screening assessments, make appropriate referrals, connect clients to social services, and keep up to date case files. The case manager will carry a caseload of no more than 15 families.

Highlighted Duties and Responsibilities

  • Work with families to develop monthly case management plans which incorporate goals for both adults and children related to core goal areas, such as stable housing, family stabilization and support, etc.
  • Meets weekly and as needed with families on caseload to develop case plans, review progress, and follow-up on tasks and services
  • Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate
  • Completes required assessments with families
  • Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs

Minimum Qualifications

  • Bachelor’s Degree in human services field preferred
  • Minimum of two years of experience in social services or case management
  • Knowledge of community resources
  • Strong communication skills to include verbal and written
  • Knowledge of HRSA, HIPAA standards and CAHP process

Lead Case Manager

Hours- Monday-Friday 9:00am-5:30pm (one Saturday out of the month)


The Short Term Family Housing (STFH) Lead Case Manager Supervisor will report directly to the Director of Residential Services and will be responsible for providing supervision, administration, and oversight to the junior case manager. Additionally, the lead case manager will be required to carry a caseload of no more than 15 families and provide case management services.

Highlighted Duties and Responsibilities

  • Provide case management services to ensure that families in the STFH facility are receiving necessary comprehensive support services
  • Assists families with finding stable housing and achieve stability
  • Provide oversight and administration of team functions and guidance to junior case manager
  • Perform initial intakes, which includes welcoming families to the program, reviewing policies and procedures with them, and completing psychosocial assessments
  • Serve as the first point of contact for case managers and in assessing and planning how to address client crisis and Unusual Incidents that arise
  • Review Unusual Incident Reports (UIRs) from case managers prior to submission to ensure its completion and compliance
  • Participate actively in clinical supervision to ensure that client goals are met and services are provided in a clinically appropriate and ethical manner

Minimum Qualifications

  • Bachelor’s Degree required; Master’s degree preferred
  • Must have a minimum of five years of professional experience providing counseling and case management services to individuals/families experiencing homelessness or other related populations
  • Basic knowledge of child development and effective programming for children
  • Knowledge of community resources
  • Valid Driver’s license and reliable transportation
